In addition to submitting the form online, the following documents must be enclosed:
- The nominees CV - in English and Hebrew.
A brief abstract of no more than 250 words. - Field of Research describing the nominee's specialization and professional achievements.
Arts and culture nominee: Achievements and creations. - Education and degrees.
- Key roles the nominee held or holds.
- List of publications.
- Awards the nominee received.
Please list prize name, organization awarding, when and where awarded. - Letters of recommendation
At least three letters from three different professionals (Required).
Appendixes:
You may add additional material for your nominee: books, magazines, articles.
This material will not be returned. Please do not send originals.
